Please arrive on time for your visit! Your intake process, registration and health screenings will take place prior to your procedure with the Doctor.

Our Nurses will review your medical history with you at the time of your visit, please make a list of your health conditions. Your Safety is our priority, please bring a family member or friend with you if you need assistance with recalling your medical history accurately.
